What is Avibactam/NXL-104?
Avibactam is a non-β-lactam β-lactamase inhibitor developed by Actavis (now Allergan) jointly with AstraZeneca. A new drug application for avibactam in combination with ceftazidime (branded as Avycaz) was approved by the FDA on February 25, 2015, for treating complicated urinary tract (cUTI) and complicated intra-abdominal infections (cIAI) caused by antibiotic resistant-pathogens, including those caused by multi-drug resistant Gram-negative bacterial pathogens.

The -lactamase serine nucleophilic attack avibatanamide bond, open loop to form a covalent bond, the enzyme - inhibitor complex, as an enzyme inhibition form, without hydrolysis, and then cyclization to form a lactam ring to get Avibatan Chemicalbook tan. The nucleophilic attack leads to the rate of ring-opening much higher than that of cyclization, resulting in the inhibition of-lactamase. In this process, the structure of Avibatan itself can be restored by the reverse reaction, so it has a long-term inhibitory effect.
It is one of the most promising new -lactamase inhibitors. Compared with the three listed -lactamase inhibitors, they are long-acting and covalently binding to enzyme reversibility, and do not induce -lactamase production.
